What is TeleSNF in healthcare?
For patients in skilled nursing facilities (SNF), Premier Health’s TeleSNF increases access to board-certified physicians and accelerates delivery of care, with the goal of avoiding unnecessary transfers to an emergency department. Through video consultation, physicians assess a patient’s needs and make care recommendations. Using TeleSNF, skilled nursing facilities gain virtual face-to-face access to board-certified physicians during nights, weekends, or other times when additional providers are needed.
